Annual Declaration of Compliance for California Based on a good faith understanding of the statutory requirements, the Comprehensive Compliance Program of Lifeward Inc. (“Lifeward”) meets the requirements of California Health and Safety Code sections 119400- 119402. Lifeward’s Comprehensive Compliance Program is aligned with (1) the United States Department of Health and Human Services, Office of Inspector General’s April 2003

“Compliance Program Guidance for Medical Device AdvaMed Device and Diagnostics Manufacturers” and (2) the AdvaMed Research and Manufacturers of

America’s “Code on Interactions with Health Care Professionals.” Lifeward requires all employees to comply with its Code of Business Conduct and Ethics and U.S. Healthcare Compliance Manual. Lifeward’s Comprehensive Compliance Program has been designed to prevent and detect violations of company policy or law. In the event Lifeward becomes aware of potential violations, it will, where appropriate, take steps to investigate the matter, pursue disciplinary action, and/or implement corrective measures to prevent future violations. Lifeward has established an annual aggregate dollar limit of $2,000 per California licensed healthcare professional for the following items: (1) meals provided in connection with educational presentations or scientific exchange and (2) educational items intended to enhance patient care.
